CladeHealth™ Tracker Personal Health Record Tool
CladeHealth™ Tracker is a mobile application (app) that lets users intuitively track all aspects of their personal health. Available now on Apple’s App Store and coming soon to the Android Market, CladeHealth Tracker is a full electronic system that keeps health records in one coherent place to replace the haphazard paper-based system with data in multiple locations. This simple application lets individuals have fully informed conversations with their healthcare professionals.
Users can enter information on healthcare providers, health conditions, lab results, medications, surgeries, upcoming tests, and emergency contact info. Because typing all of this into a smart phone can be intimidating, Aegis has developed ways to make data entry less tedious and time-consuming:
- Through CladeHealth Tracker’s Desktop Data Entry system, users can enter names and data via their computer keyboard, and their entries will be wirelessly “pushed” into the mobile app. No extra software required. No additional cost.
- After opening the app, users have the option of clicking icons to enter data or using a Wizard to step them through initial data entry.
- With a built-in database of health conditions and medications, users can type just a few letters into a field and bring up a list from which to choose. Bad spellers, rejoice!
The application even lets users generate a report to email or print out for a family member or new physician.
CladeHealth™ Management Enterprise Platform
CladeHealth™ Management is a complete communication system that integrates CladeHealth Tracker with an enterprise-strength server system to allow for remote monitoring of patients in clinical trials and registries. CladeHealth Management expands the functionality of CladeHealth Tracker through Extensions, which allow for real-time collection of disease-specific information—from patient-reported outcome questionnaires to device-based data capture.
